- Identification: Two (2) forms of current, not expired official signature, Gov’t photo-bearing ID
- Identification: (Photo ID) required for admission: Driver’s License, DMV ID card, Passport and/or Military ID
- Identification: (Social Security card) must be a U.S. government-issued, signed and non-laminated
- High School transcript (showing proof of graduation), College Transcript (showing proof of graduation) or GED transcript (showing GED test scores). Penn Foster and Cornerstone transcripts are accepted
- CPR -BLS for Healthcare Providers (AHA) –may be completed at the school for $45 (student discount price) – (Not required to attend class but required to attend clinical rotation)
- Criminal Background $45 – (per applicable clinical rotation program)
- Scrubs: Burgundy and/or Grey – (required to be purchased at the School – $35/set for top & bottom
- Closed-toe black shoes – (may be purchased at the school – required to attend clinical rotation)
